How it works

1. After purchase, you'll receive a link to your intake form.
2. Complete it online in about 15 to 20 minutes. It only shows you the questions that apply to you, and there's nothing to download or send back.
3. Within 10 working days, you'll receive your written report as a PDF, personally reviewed and signed off by Kelsie, NMC Registered Nurse Prescriber, PGCert.

Your report includes

• your overall status, whether you're opening soon or already open;
• what's missing, why it's needed and how urgent it is;
• what to confirm with your insurer, regulator or local authority;
• what you already have in place.
